Webinar: Population Health and Social Determinants of Health

Population Health and Social Determinants of Health (SDOH) are increasingly popular terms within the health care industry—but what do they mean? Why are they important? Population health is the health outcome of a group of individuals, including the distribution of such outcomes within the group. SDOH are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks.

Why are these factors important in the overall health and wellness of the population? They contribute to wide health disparities and inequities, which increases the risk of certain severe and life-threatening diseases. In our latest webinar with Kristine Snyder, Senior Director, Solutions Engineering, we discuss SDOH and Population Health, and how home and community-based care (HCBS) providers can identify issues early in the care continuum resulting in early intervention.
