Sandata Technologies, LLC (“Sandata”) is committed to maintaining robust privacy practices for its users. This privacy statement (this “Privacy Statement”) summarizes Sandata’s practices with respect to the privacy of users of Sandata’s website (the “Website”) or Sandata’s applications (the “Applications”). From time to time, this Privacy Statement is subject to change.  For example, as we update and improve our products and services, new features may require modifications to this Privacy Statement.

What is personally identifiable information?

“Personally identifiable information” is any data that could potentially be used to identify a particular person, such as a person’s name, address, e-mail, user ID, social security number, etc.

How is information collected, used, and disclosed?

We may request that you voluntarily supply information within the Applications in order to provide services or to enhance your experience.  Providing this information is up to you, however if you chose to do so, it must be sent in an encrypted fashion that sufficiently protects PHI/PII.  Mechanisms to send data securely vary depending on the sender’s electronic mail system, network and system configuration, etc., and are the responsibility of the sender.  We do not collect any personally identifiable information without your consent.

Sandata is a Business Associate performing services on behalf of Covered Entities. As such, your PII may be provided to Sandata in order for Sandata to provide the appropriate services in accordance with contractual obligations to our customers. Requests for updates or deletion of PII must be coordinated through the Covered Entity that collected the information, as such requests may impair Sandata’s ability to provide agreed-upon services.

If you have voluntarily provided information, you consented to the collection and use of your personally identifiable information as described in this Privacy Statement. In some circumstances, Sandata may share information you voluntarily provide us with trustworthy business partners. We do not sell or rent personal information collected through the Applications. Sandata may also provide certain statistics about site visitors, including volume, traffic patterns, and related site information to reputable third-party vendors, but these statistics will not include any personally identifiable information.

Sandata may also disclose personally identifiable information in special cases when we have a good faith belief that such disclosure is necessary to: (a) comply with legal requirements; (b) defend and protect company property; (c) enforce Terms and Conditions of Use of the Applications; or (d) protect the interests of our users or others.

Visiting the Site from outside the U.S.

The Applications are operated in the United States. This Privacy Statement is intended to cover the collection of information within the Applications from United States residents only. If you are located outside of the United States, please be aware that any information you provide to us will be transferred to the United States. By using the Applications, participating in any of our services and/or otherwise providing us with your information, you understand and agree that your information may be moved to, accumulated and processed in the United States where our servers are located and our central database is operated and such information may be transferred to our third parties with whom we share such information as described in this Privacy Statement.

Children’s Policy

The Applications are restricted to individuals eighteen (18) years of age and older.  The Applications are not designed for or targeted at children. No one under the age of thirteen (13) may provide any information on the Website, within our Applications. While our Applications might be used in connection with services provided to minors, an adult guardian is responsible for entering any appropriate information into the Application.  We do not knowingly collect and maintain information from children under the age of thirteen (13). If we are informed that a user is under the age of thirteen (13), we will immediately close that user’s account and delete such information. Should you have knowledge that a user is under the age of thirteen (13), please contact us immediately.

How do we protect your information?

We are painstaking in our efforts to protect your personally identifiable information. Sandata has security measures in place to protect the loss, misuse, and alteration of the information under Sandata’s control. Sandata employs strict security measures to safeguard personally identifiable information. This includes, among other things, using firewalls, encryption, intrusion detection and site monitoring. Unfortunately, no data transmission over the Internet can be guaranteed to be 100% secure, so despite our efforts, we cannot ensure or warrant the security of any information you transmit to us or receive from us.  Internally, we will restrict access to your personally identifiable information to employees who need access to the information to do their jobs. These employees are committed to our privacy policies.

As a provider of services to the healthcare industry, Sandata has implemented programs to address the transaction standards, and the privacy and security implications of the rules promulgated pursuant to the Health Insurance Portability and Accountability Act of 1996 with respect to protected health information.

How can you help protect your information?

If you are using the Applications for which you registered and created a password, we recommend that you never divulge your password to anyone. We will never ask you for your password in an unsolicited manner. Also remember to sign out of the registered site and close your browser window when you have finished working. This is to ensure that others cannot access your personal information and correspondence if others have access to your computer.

What are cookies and how are they used?

The Website, like most other commercial websites, may utilize a standard technology called “cookies” to collect information about how our site is used. Cookies help a website operator determine that a user had visited the site previously and save and remember any preferences that may have been set by the user while browsing the site.
